Two ways to enter the clinic:

1. Go to EQentries.com and register online and pay using a credit card.  Go to the clinics calendar page to start here:

Please note- you must first signup for EQEntries as a user in order to be able to see the signup link for the clinic as shown below:

Then go to the signup link under the login module: 




2. You may print out and fill out the paper form and send to Pamela Byers per the instructions on the form and send payment by check.  Download the registration form here:
